If you are pre XP you may want to try WindowsBlinds although it isnt free.
I think WindowBlinds can do some additional stuff since it replaces the entire API, but XP's theming is based on the same code (I believe they recruited StarDock) just native, and much faster. It was the StyleXP guys who released the uxtheme.dll hack (and their commercial program uses it, so they're killing their own business...wtf)WindowBlinds...pah
You can hack XP's built-in themes manager to do all the stuff that WindowBlinds does (same thing without paying, basically.) It's out there on the Inter Net somewhere how to do it, although it does mess up your Add/Remove Programs thing.
