I seem to recall that Mac computers seem to mess up SD cards as to make them unusable in a Pandora. Can you get a friend to format it on Windows maybe?
Create your custom category from the Manage Custom App Category menu.
Then you have to find the app you want and highlight it, then press Space and choose Recategorize, then choose your custom category.
After that it should appear in a new tab. Now rinse and repeat for the other apps you want.
I know that sometimes a USB device can crash the USB system, so maybe a reboot isn't resetting it enough? It's a long shot but you could try leaving the battery out for 20 minutes, then try again.

Regarding the folder problem: if you are using the default Thunar file manager then a bug won't let you go down more than a couple of levels into subfolders. You should change your View type from "icons" to "detail" or vice versa (dont remember right now)
