Ah, you are right! I have been working on that for a too long time to remember such details :)
There are 8 unused solder pads on the battery oriented side of the CPU board where exactly those shared pins are available we need :)
Otherwise, if custom made, it should be easy to rip an SD->Micro-SD adapter apart and solder a small PCB to the traces.
the benefit of building an SD-card shaped JTAG adapter is that it can be applied to any Pyra without opening it.

A problem is that these contact pins are not very stable unless sealed inside the adapter.
I would simply make an 1.6mm PCB having the standard SD card shape and contact pads at the right locations. Similar to this:
