A recent Destructoid article about the Bioshock intros refreshed my memory of a discussion I had with a pal recently on our favorite game intros.
Some of mine include..

Fahrenheit (aka Indigo Prophecy)
Day of The Tentacle
Half Life
Legacy Of Kain: Soul Reaver
Pathologic
Portal 2
Bioshock
Final Fantasy VII

I've loads more but I'll leave it there for the time being

Although I'm ignoring the company since their always online bs, I'll still have to go with "everything from Blizzard".
 
Garrett started the thread with several really good ones. Let us not forget about the fallout series.
Also, Ultima Underworld, Dune II, Red Alert, Lands of Lore, Oblivion, Conquest of the Longbow.

Sadly, there's no decent video of its intro on YouTube, the Dreamcast version added some extra cheese over the Neo-Geo ones. It's amazing.

I mostly play arcade style games, so there are not that many I frequently see. Some that stuck in my mind from the last years are Witcher 3 and Stick it to the Man, as a kid I loved the Warcraft 3 intros. Soul Calibur also looks amazing for its time period, Super Smash Brothers Melee, too.

Edit: Oh, and my personal award for the stupidest intro goes to Ecco the Dolphin: Defender of the future. When I saw it for the first time and didn't expect it at all, it just seemed like ridiculous ravings of a drug-addict read by a waaaay to serious narrator. I can't believe I spend 15€ on a White Label version of this crappy game... At least me and a friend of mine who was with me had a good laugh :D

The games themselves usually determine how much I like the intros, so my list is nostalgia-based and way too long. Many great ones were already mentioned.

most fighting game intros (3D ones in particular) are the same, but I do enjoy them

A couple spring to mind

Ghosts n Goblins on SNES
With excellent music, smashing glass SFX, Mode7 zooming effects, a girl kidnapped, and an overview of the games map ... It has it all

Operation Wolf arcade
Is pretty simple, but it's so iconic to me as played the arcade of it a lot growing up.
The intro graphics and music get you pumped for the game ahead.
Plus it always reminds me of Arnie in Commando as that film also does cuts showing him getting armed up when he raids that arms factory
